Report of Findings
As a bonus, you will have the opportunity to receive a complimentary Report of Findings consultation with Dr. Henze (either the next business day or the one following), where he will personally review the following with you:
- What he found in your exam and on your x-rays
- How he feels he can help you, along with options for either Relief Care only or Corrective Care
- How long he estimates each of those options will take
- His best estimate of what the cost for each of those options will be for this and what insurance (if any) might pay
Qualifications and Restrictions
To qualify for this special you must:
- Pay $99 on or before your initial visit to Henze Chiropractic & Wellness
- Agree that these services will not be billed to any insurance company for payment consideration.
Please Note
If you have insurance that will cover our normal charges for your initial examination, x-rays and treatment, where the part you would be required to pay is less than $99, we will be happy to bill your insurance(s) for you, and only require you to pay the patient / insured / subscriber portion due.
The most you will be required to pay for your initial visit while this Internet Special is being offered is $99.
